Add this one: Poor reward structures for key employees. If you dont take good care of your resources, sometimes they wither.

Poor management, bad attitude sure - don't pop off at your boss unless you have something else lined up and you can afford to take that risk...Down-sizing is good, not as bad as it was in the 90's though, Economic shifts like major changes in manufacturing, technological advances...and here's a brutal one, right or wrong, a President who supports shipping jobs out of the country.

Work related injuries. Sleeping on the job. Harm to others. Drugs or Alcohol.
