The answers to your two questions are YES & YES. You have to include it into your tax return as income whether or not the referal bonus has been included into your employer's T4 or T4A given to you.

Absolutely.

Section 5 of the Income Tax Act brings into your income any benefit on account of your employment. A referral bonus is such a benefit and is taxable. There are no exclusions in the legislation for such amounts.

All revenue earned is potentially taxable.

If the money is earned from a business they must account for it and you should receive a T-4 slip with the amount included.
