I did not receive my T4 tax slip- I live in Ontario, CANADA.?

How do I report my my past employer even though I've sent them my address update back in December of 2007 and also called beginning on March for my T4 slip and they told me it'd be here by 2 weeks and it wasn't.

If you are looking for the information from your T4, you can call CRA at 1-800-959-8281 (press *). Hopefully it is in their computer by then.

Employer suppose to send you T4 tax slip by end of Feb. but in your case it was not.

You could check your last pay stub that will tall you YTD (Year to date) earnings, CPP and EI, Tax deducted at source payments you could use that to calculate the tax.

Report them to Revenue Canada. They will be fined $100 a day that you don't receive your form up to $2000
